Refactor: Consolidate defaultStatusText().

Response::constructor() included fallback code to set status text based on which range the code is in, if we didn't have a specific string for the specific code. This could be moved directly into `defaultStatusText()`, which helps with my next commit.

There are only two call sites of this function, and the other call site is cleary not affected since it is only called for certain specific knows codes. In order to prove there are no other call sites, I moved the declaration out of the header.
