Improve tracing for MemoryCache

- rename `span` to `readSpan` in several places where appropriate for readability.
- extend readSpan to after fallback is complete.
- use proper span tree structure for spans
- remove redundant fallback_complete span and merge into fallback span
- Fix UaF in sharing span between fallback failure and success cases
