[crypto] Fix overwriting AES-CTR counter in case of overflow

Addresses internal issue EW-6631. Fixes the counter overwrite so that the
initial input counter is preserved. Updates comments that were highly misleading
and made it seem like this code was sound. Adds a regression test.
