Add exception handling for tail stream JS failures

- Add TAIL_STREAM_JS_FAILURE exception detail type to identify JS exceptions
- Convert JS exceptions to KJ exceptions with proper detail marking
- Handle tail stream failures by returning EXCEPTION outcome
- Preserve all exception details when converting from JS to KJ
