Add tracing spans to memory cache

Added comprehensive tracing throughout the memory cache to help debug
performance issues and understand cache behavior in production.

The main changes:
- Track cache hits/misses with size metrics
- Show when requests get coalesced (multiple requests waiting on same key)
- Measure how long fallback functions take
- Log evictions with reasons (expired vs LRU)
- Monitor write rejections and why they happen

This should make it much easier to debug cache issues and understand
why certain requests are slow.
