Replace HashSet<ExternalMemoryTarget> with a singleton ExternalMemoryTarget.

AFAICT there is no need to allocate a separate ExternalMemoryTarget for every adjustment. This saves quite a bit of overhead when using adjustments -- previously, a mutex lock and a HashSet operation were needed whenever creating or destroying one.
