Tracing cleanups

Clean up some unused APIs even if some of which might be restored later
as needed.
Remove the ability to create *only* a user span.
Remove the ability to add a tag *only* to the user span.

This is strict on purpose.
We have seen that in practice, whenever a span or a tag are only
added to the user span, we actually want them to be added to both
the user and the internal trace.

If we ever have a really good reason to add a tag or a span
only to the user trace, we can always add it back.
