Add README and AGENTS.md (with CLAUDE.md symlink)

README documents what the app does, how to run it, the keyboard shortcuts,
and the three-layer architecture. AGENTS.md captures repo conventions and
invariants for agents (single-reducer state, preview/export parity, worker
boundary, cross-origin-isolation constraints). CLAUDE.md symlinks to AGENTS.md.
