Simplify publisher session lifecycle

Discard replaced or expired publisher generations without waiting for SFU cleanup. Preserve cleanup and controller permissions within the current generation and improve SFU error diagnostics.

Cover replacement, expiry, stale requests, and failed allocations. Validated repository checks with the CI Rust toolchain, 23 Worker tests, the production bundle, and a real board reboot with browser audio and spectrum reconnection.
